hi everyone I'm 7 Day Cream assistant principal here at Castro Valley High School and like to take just a minute to show you how you can log into our Frontline Health Portal system to complete the daily Health screener for your student before return to Camp so I'll just start by visiting our website and under the four families drop down menu you'll see this one titled Frontline Health Portal click there and I text me to the login page as a staff member I'll need to go through single sign-on and your account may be linked to a Google profile as well once you've successfully logged in you'll see that you've got user information and a to-do list with a high priority daily covid-19 health check so the complete the form I'll go over here to this icon quick there metal open up the questionnaire in the past 10 days has my student been diagnosed with covid-19 know in the past 14 days as my student have close contact with anyone. Diagnosis covid-19 Pinellas well since last at school or work has my student displayed any of the following symptoms and I'll just take a quick look at these symptoms make sure that's all I know and everything else is all cleared will just check this bottom box and submit the form your papa and bright green health check cleared for 2020 and that means my student is all set to head on up to campus and join us for in-person instruction I'll show you how you can do it on your phone as well I'll just log on select the daily screener scroll through the questions and submit and I'm all cleared play just once reminder that the daily health screening is something that we'd like for you to complete every day of the week 7 days a week so that we can track the health of your student and make sure that they're all good to return to school on the designated days so please make it a habit do it every day and we really appreciate your help here in Trojan togetherness
